I agree that Shortstop, LLC is not responsible for accidents and/or medical or dental expenses received as a result of my child(ren) participating at the camp. I understand that no reduction in the tuition will be made for late arrival, early departure, vacations, illness or injury. Permission is herby granted for photographs and/or videotapes to be taken of my child(ren) at the camp. Shortstop, LLC has the right to utilize these photographs in the camp brochure or displays, or for any other purpose. In the event that I cannot be contacted in an emergency, I hereby grant Shortstop, LLC permission to give whatever immediate treatment in necessary and/or take my child to a hospital or other medical treatment facility.
